Once you have made an appointment to see Dr Rooney, please download your appropriate patient registration form. For example: If you have ‘hip pain’, download the  ‘Hip Registration Form’, print and then fill it out.

Please bring to your first appointment your completed ‘Registration Form’, G.P referral and any X-rays or imaging of your painful area. If you do not have a printer you will be provided with a ‘Patient Registration Form’ upon arrival to your appointment. Please arrive 10 mins prior to your appointment with Dr Rooney.
